IMPROVING ACCESS TO BEHAVIORAL HEALTH CARE

Catholic Charities, Diocese of Trenton was one of the first seven organizations in New Jersey to be designated as Certified Community Behavioral Health Clinic (CCBHC), This innovative new health care model that will transform the delivery system of behavioral health by addressing socioeconomic conditions.

Introduced by the Substance Abuse and Mental Health Services Administration (SAMHSA), CCBHCs are designed to build an infrastructure that includes the full circle of community-based health assets in the continuum of care for the most vulnerable among us. In addition to improving access to behavioral health care and substance use treatment, CCBHCs further improve overall health by linking individuals to physical health care and social services.

As a CCBHC, we:

• Ensure rapid access to integrated behavioral health services and substance use treatment, including 24/7 crisis response and medication-assisted treatment.
• Offer services and treatment to individuals who do not have insurance.
• Provide person-centered treatment planning.
• Offer case management, peer and family support services.
• Coordination with healthcare and social service providers.
